2008-09 EMU Medical Eligibility Forms

PLEASE NOTE: Athletes: Be aware that there are two medical documents you need to complete and have on file as an EMU student-athlete. The first form for athletes is the Medical Eligibility Packet found at this site or attached to the packet. For all Fall sport athletes, please complete and return the completed packet to the address listed by August 15th. All athletes on other teams must complete and return the packet one week prior to the first practice session of your season.

The second medical form is the Student Health Evaluation Form issued by the University Health Center. If you did not receive this with your original admissions packet, click here and go to the Student Health Evaluation Form. While in most cases this form does not require a physical exam, it is required by the University.

Athletes are required to repeat the Medical Eligibility each year whereas the Student Health Evaluation Form is only completed upon enrollment. Returning athletes do not need to do this if they have previously returned the Student Health Evaluation Form. Complete and return both if this applies to you. The two documents are not the same, and one will not be accepted for the other.
